Hospice Care: At your home, your nursing facility,
or at Coastal Hospice at the Lake.
Hospice
assists family members in caring for persons in the last phases of
an incurable disease so that they might live as fully and
comfortably as possible. This specialized health care is given
primarily in the home. The emphasis is on control of pain and other
distressful symptoms.
Services are provided in accord with written orders by the patient's
own physician. Twenty-four hour on-call RNs. Respite care and inpatient services can be
coordinated.
Our
professional team of caregivers can provide quality end-of-life care
to patients in Dorchester, Somerset, Wicomico, and Worcester
Counties in Maryland regardless of ability to pay.
Nursing Care
Quality nursing services help to provide comfort to patients
suffering from a serious illness. Our professional staff of
registered nurses work with the patient's personal physician to
develop plans of care. Holistic palliative care is coordinated with
our aides, counselors, volunteers, and therapists to ease the
journey toward the end of life.
Home Health Aide Care
Under the supervision of RNs, aides provide personal care to
patients. Essential daily activities such as feeding, bathing, and
grooming provided by a caring aide can help the patient feel more
comfortable. Aides help to ease pain and provide companionship for
patients.
Social Work
Trained counselors assist patients and families in coping with the
challenges of illness. Social workers listen to concerns, answer
financial questions, and provide links to community resources. After
an initial visit by a social worker, services are available to
patients and their families.
Pastoral Counseling
Spiritual care often helps patients and families cope at the
end-of-life. Our Pastoral Counselors are available to provide
support regardless of religious background and in conjunction with
the patient's church if desired.
Bereavement Care
Someday we must all face the grief of losing a loved one. Although
grief is a natural and universal process, our response to it is
deeply personal. Understanding our shared human and individual
responses to loss is essential in attaining a peaceful spiritual,
physical, and emotional balance. The Bereavement program offers
support for family members of Coastal Hospice patients for about one
year after their loved one's death. The program includes person to
person contact, group support, bereaved children's support, and an
annual memorial service to remember loved ones.
Therapy
Physical, occupational, speech, or music therapy can be provided to help
relieve suffering from a disabling illness. Pain management
techniques taught by licensed therapists can improve the patient's
quality of life.
Volunteers
Trained volunteers offer additional support, comfort, and companionship.
